Pituitary Tumors: Benign vs Malignant Explained

The Core Distinction: Benign vs. Malignant

In general oncology, tumors are broadly categorized into two types: benign (non-cancerous) or malignant (cancerous). When examining the pituitary gland, this distinction is both highly distinct and uniquely nuanced.

Benign Pituitary Tumors (Adenomas)

The overwhelming majority of pituitary growths—roughly 99.9%—are benign epithelial tumors known as pituitary adenomas.

  • Behavior: These tumors grow slowly and remain strictly localized within the sella turcica or expand gently into immediate neighboring tissues, such as the optic chiasm (the intersection of the optic nerves).
  • Characteristics: They do not metastasize—meaning they do not break away to colonize the lungs, liver, bones, or other distant organs.
  • Clinical Impact: Despite being non-cancerous, they are not entirely harmless. Because the skull base is a rigid, unyielding space, an expanding benign adenoma can exert physical pressure on the remaining healthy pituitary tissue and nearby brain structures, leading to significant hormone deficiencies or neurological symptoms.

Malignant Pituitary Tumors (Pituitary Carcinomas)

True malignant tumors of the pituitary gland are classified as pituitary carcinomas.

  • Rarity: They are exceptionally rare, representing less than 0.1% of all primary pituitary tumors.
  • The Defining Diagnostic Fact: Structurally and microscopically, a pituitary carcinoma can look identical to a benign adenoma under a pathologist’s microscope. Therefore, an endocrinologist or neurosurgeon cannot diagnose a malignant pituitary tumor based on a localized biopsy alone. Instead, a pituitary tumor is classified as malignant only when it demonstrates metastasis—spreading through the cerebrospinal fluid (CSF), lymphatic system, or bloodstream to seed tumors in the brain, spine, lymph nodes, or distant organs.
  • Behavior: These tumors are highly aggressive, exhibit unpredictable growth patterns, and are highly resistant to standard therapies.

Classifying Benign Adenomas: Size and Secretion

Because benign adenomas comprise nearly all clinical cases, endocrinologists classify them further using two distinct parameters: their physical size on high-resolution imaging and whether or not they actively manufacture hormones.

  1. Classification by Dimension: Micro vs. Macro

The physical boundaries of a tumor dictate the structural complications a patient might face.

  • Pituitary Microadenomas: These are small tumors that measure less than 10 millimeters (1 centimeter) in diameter. Because they are small, they rarely put pressure on nearby brain structures or cause visual changes. They are usually discovered because they overproduce a specific hormone or are found accidentally on a brain scan ordered for an unrelated issue, like a concussion or migraines.
  • Pituitary Macroadenomas: These are larger tumors measuring 10 millimeters or greater in diameter. As a macroadenoma expands, it can outgrow the sella turcica and compress the healthy portion of the gland, causing it to fail—a condition known as hypopituitarism. It can also push upward against the optic chiasm, resulting in progressive vision loss.
  1. Classification by Function: Secretory vs. Non-Functioning

The cellular makeup of an adenoma determines how it affects the body’s chemistry.

  • Functional (Secretory) Adenomas: These tumors are made of specialized hormone-producing cells that operate completely outside the body’s natural feedback loops. They pump excess hormones directly into the bloodstream, leading to specific medical conditions based on the hormone involved:
    • Prolactinomas: Overproduce prolactin, causing irregular or absent menstrual periods, unexplained breast milk production (galactorrhea), and infertility in women, alongside erectile dysfunction and low testosterone in men.
    • ACTH-Secreting Tumors: Excess adrenocorticotropic hormone triggers the adrenal glands to flood the body with cortisol, resulting in Cushings Disease (characterized by rapid upper-body weight gain, a rounded “moon face,” high blood pressure, and fragile skin).
    • Growth Hormone-Secreting Tumors: Excess growth hormone causes Acromegaly in adults, which leads to the gradual enlargement of the hands, feet, jaw, and internal organs, alongside severe joint pain and cardiovascular strain.
  • Non-Functioning Adenomas: These tumors do not produce any hormones. Because they do not cause sudden chemical imbalances, they often go unnoticed for years. They are typically diagnosed only after they grow large enough to cause physical pressure symptoms, such as headaches or vision impairment.

How Pituitary Tumors Manifest: Symptoms to Watch For

The signs of a pituitary tumor generally fall into two broad categories: symptoms caused by hormone imbalances and structural symptoms caused by the physical size of the tumor (known as “mass effect”).

Structural “Mass Effect” Symptoms

When a macroadenoma grows upward and out of its bony compartment, it encounters the optic nerves. This localized pressure can cause specific neurological signs:

  • Bitemporal Hemianopsia: This is a classic visual pattern where pressure on the optic chiasm damages the nerve fibers responsible for peripheral vision. Patients experience a progressive loss of the outer halves of their left and right visual fields, often describing it as “tunnel vision” or bumping into doorframes.
  • Persistent Headaches: Deep, dull headaches located behind the eyes or in the center of the forehead can happen as the tumor stretches the sensitive lining around the gland (the dura mater).
  • Hypopituitarism: As the tumor expands, it can crush the remaining healthy pituitary tissue. This sudden loss of healthy tissue shuts down the production of other essential hormones, causing severe fatigue, unexplained weight loss, intolerance to cold, and low blood pressure.
Systemic Hormonal Symptoms

If the tumor is functional, it can cause a wide variety of symptoms throughout the body. For example, a prolactinoma might present as a complete loss of libido, while a growth-hormone-secreting tumor can slowly change a person’s facial features and increase their shoe or ring size over several years.

Modern Treatment Options

The choice of treatment depends on the tumor’s size, its hormonal activity, whether it is affecting your vision, and whether it is benign or malignant.

  1. Medical Management (The First Choice for Prolactinomas)

Unlike most brain-region tumors, the most common type of functional pituitary tumor—the prolactinoma—rarely requires surgery. Instead, it responds remarkably well to oral medications known as dopamine agonists (such as cabergoline or bromocriptine). These medications target specific receptors on the tumor cells, effectively stopping hormone overproduction and shrinking the physical size of the tumor over time.

  1. Surgical Intervention

If an adenoma is pushing on the optic nerves, causing progressive vision loss, or overproducing hormones that cannot be controlled with medication (such as in Cushing’s disease or acromegaly), surgery becomes necessary.

  • Transsphenoidal Resection: This is a minimally invasive procedure and the gold standard for surgical removal. A specialized neurosurgeon reaches the pituitary gland by passing advanced endoscopic tools through the nasal cavity and the sphenoid sinus. This approach eliminates the need for an open craniotomy, avoids visible external scars, and shortens recovery times.
  1. Radiation Therapy

For aggressive benign tumors, growths that cannot be completely removed with surgery, or tumors that return after treatment, we use targeted radiation therapy. Options like Stereotactic Radiosurgery (Gamma Knife) deliver a highly focused, single dose of radiation directly to the tumor tissue while sparing the healthy surrounding brain matter.
